Help with a Federal Agency

If you cannot get an answer from a federal agency in a timely fashion, or if you feel you have been treated unfairly, my office may be able to help resolve a problem or get you the information you need. While we cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response to your problem.
Residents of the 5th Congressional District of Michigan can contact us for assistance in dealing with Federal agencies. Clicking the link below will allow you to electronically fill out, sign, and submit the required Privacy Act waiver form that is the first step in this process.
Please include all pertinent information and claim numbers in your correspondence—such as:
- Your Social Security number
- VA claim number if applicable
- Taxpayer identification number (Social Security number, if individual) for an Internal Revenue Service problem, etc.
- Your address, phone number and email so that we can obtain any additional information from you that might be necessary (read more below)
- Copies of any related documents or correspondence that you may have from the agency involved
- Explanation of the problem and summary of your desired outcome

Necessary Documents to Process Your Request
Please read the following list to see if any extra documents will be needed for your case and attach them to your inquiry and the Privacy Release Form.
Department of Defense (DOD) and Defense Finance and Accounting Service (DFAS) Case
- All Branches: Any request for new awards please contact the office to find out what forms need to be completed to process your request.
- Retired and Annuitant Pay:Death certificate, direct deposit form, W-4, annuitant’s SSN, SF-1174 for arrears of pay (Please note, DFAS and the Veteran’s Administration are separate agencies).
- Debts and Claims: Copy of debt notices, DD Form 2789, Waiver/Remission of Indebtedness Application, 20140917 draft. The Department of Hearing and Appeals (DOHA) is a separate agency from DFAS.
- TIRECARE (Humana Military): Please complete out DD form 2870.
National Archives and Record Administration (NARA) Case
- All Cases: Please complete the Standard Form 180 - Request Pertaining to Military Records.
- Discharge Upgrade or Correction to Military Service Record: Please complete DD-149.
United States Department of Veterans Affairs (VA) Case
- The VA does not accept non-VA Powers of Attorney (POA) please complete VA form 21-22a Appointment of Individual of Claimants Representative.
- CHAMPVA: Please review the CHAMPVA Guidebook. If you have any questions, then please contact the office.
- Headstone or Marker request: Please complete the VA Form 40-1330.
Office of Personal Management (OPM) CASE
- All Cases: Please include a CSA/CSF number for your claim and a full SSN.

The Privacy Act of 1974 (5 U.S.C. § 552a) requires that Members of Congress or their staff have written authorization before they can obtain information about an individual's case. We must have your signature to proceed with this type of request.
